Adherence after renal transplantation: challenges related to patients with psychiatric disorders

Introdução


Psychiatric symptoms may influence receptors of organ transplantation especially in post-transplant adherence, self-care, immunossupression maintenance treatment, and graft outcomes. Literature highlights the presence of active psychiatric symptoms as predictors of negative organ transplant results.

Material e Método

A report of a woman who had underwent kidney transplantation.

Resultados

We present a 50-year old female, who received a kidney transplant and lost the graft due to an active psychiatric disorder. At first moment, she didn´t tell the health team about her disorders. After the transplantation, she didn´t take any medicine. She was referred to immediate hospitalization, admitted with uremic symptoms and underwent to a graft nephrectomy. She remained with psychiatric symptoms that she had in her first hospitalization. By the graft loss, the patient was discharged and was referred again to hemodialysis. The family received orientation, emphasizing importance of psychological the and psychiatric follow-up.

Discussão e Conclusões

The case shows the challenges of transplantation in patients with psychiatric disorders highlighting the importance of psychosocial assessment, adherence, social support and health team care. Interventions in this area are poorly described in the literature and guidelines to directed the psychosocial assessment before the transplant are a growing area of interest. Also, building a bridge between the patient, his family and the health team could minimize the risk of this kind of poor outcome after transplantation.
