Título

POSTERIOR UVEITIS AS A LATE MANIFESTATION OF CHIKUNGUNYA FEVER

Introdução / Purpose

To report a case of posterior uveitis as a late manifestation of Chikungunya fever in North of Brazil.

Case report.

Resultados / Results

The patient is waiting the platelets level to raise and her clinical status to get better, in order to be submitted to posterior vitrectomy.

V.M.Q., female, 77 years old, from Amazonia region, got Chikungunya fever in june/2017. She was hospitalized, had 4 cardiac arrests, pneumonia and kidney failure. In September, she noticed blurried vision in her left eye. She was submitted to full eye examination. Her right eye had no abnormalities. Her BCVA OS was HM. Biomicroscopy revealed pseudophakia with capsulotomy and no anterior chamber reaction. IOP 10 mmHg. Fundoscopy showed intense vitreitis and an area of whitening in the upper temporal retina. She was submitted to uveitis screening, including Toxoplasma gondii, which all came negative, except Chikungunya antibody (IgG 2.9). Chikungunya IgM antibodies start to decrease within 8 days of illness. The manifestations of Chikungunya fever can be acute (3-10 days), sub-acute or chronic (more than 12 weeks). Atypical acute manifestations are more easily associated to the disease, once they occur within the period of cutaneous rash and arthralgia. Chronic ones are more difficult to correlate because of the distant period of time from the early symptoms. There are several ocular manifestations and they can occur at any time of the disease and even after its resolution. Late posterior uveitis is a rare condition and corticosteroids are an alternative of treatment, but due to her kidney status, they were not used. The only alternative was surgery. Since the Chikungunya fever is a new challenge to Brazilian ophthalmologists, it is very important to be aware of all of its forms of manifestations. There have been registered several cases in North of Brazil.
