Título / Title

ASSOCIATIONS AMONG VISUAL ACUITY, VISION-RELATED FUNCTION AND VISION- AND HEALTH-RELATED QUALITY OF LIFE AMONG PATIENTS WITH VOGT-KOYANAGI-HARADA DISEASE

Introdução / Purpose

To evaluate associations between self-reported visual-related (VR-QoL) and health-related quality of life (HR-QoL) metrics and visual acuity,disease activity,treatment and visual function measured by electroretinogram (ERG) and contrast sensibility (CS) in patients with Vogt-Koyanagi-Harada (VKH) disease followed prospectively

Material e Método / Methods

Patients with VKH disease with minimum 12-month follow-up from acute onset were evaluated.Disease activity was detected by clinical examination and complementary exams.Visual function was measured by best corrected visual acuity (BCVA),full-field ERG and CS with Pelli-Robson chart.VR-QoL was assessed by NEI VFQ-25 and HR-QoL by SF-36 questionnaires.Differences in scores of SF-26 and NEI-VFQ-25 among subgroups were analyzed with Mann-Whitney and Spearman's rho tests.Study was approved by Institutional Ethics Committee and followed Helsinki declaration

Resultados / Results

19 patients were evaluated(17F/2M;mean age 36.9±12.7y).BCVA was 20/20 in 63.1% and ≥20/60 in 94.7% of patients.SF-36 total score was 74.3(range 17-95.7) and NEI-VFQ-25 overall composite score was 80.28(range 63.9-99.4).In SF36 questionnaire,immunosuppressive therapy impacted negatively in physical and social functioning,bodily pain and vitality scores(p<0.05),while higher cumulative corticosteroids dose had negative impact in role physical and social function scores(p<0.01).Worsening full-field scotopic ERG parameters had negative effect on role physical, vitality and social functioning scores(p<0.01).In NEI-VFQ-25 questionnaire negative impact on general vision was related with worse BCVA(p<0.01),its fluctuation(p<0.05),choroidal thickness fluctuation(p<0.05) and higher cumulative corticosteroids dose(p<0.01).Lower CS had negative impact on peripheral and and distance vision

Discussão e Conclusões / Conclusion

VR-QoL and HR-QoL questionnaires with psychophysical measurements of visual impairment bring more extensive understanding of impact of choroidal inflammation and treatment in patients with VKH disease
