PURTSCHER RETINOPATHY - CASE REPORT
Low visual acuity after ocular trauma is a common complaint in emergency ophthalmologic care and has many causes. Purtcher's Retinopathy is a rare ophthalmic disease and may be caused by trauma far from the eyeball.
R.S.C.P., male, 19 years old, was referred to our Ophthalmology Ambulatory for sudden low visual acuity after an automobile accident with thoracic trauma. Denied comorbidities or previous surgeries. VA: OD hands movement / OS 20/20. Biomicroscopy normal. Fundoscopy OD: retinal whitish placoid lesions and candle flame haemorrhages in peridisc and perimacula/ OS: normal. The retinal alterations were documented with color retinography (CR) and fluorescent angiography (FA).
Abnormalities in imaging studies show the typical lesions of this rare disease, with whitish retinal plaques resembling cotton-wool spots, peridiscal and macular candle flame hemorrhages; in FA presents hypofluorescence by blockage in the areas of hemorrhage and by low perfusion in ischemic regions, besides a mild hyperfluorescence by leakage. The treatment was performed with oral corticosteroid and had a slight improvement of visual acuity after 2 months (OD: fingers count).
Purtscher's Retinopathy presents typical alterations in the retina, although it does not have fully elucidated pathophysiology, the treatment is controversial and the visual prognosis is unpredictable. When the disease occurs without a history of trauma, it is called Purtcher - like Retinopathy, and a more detailed diagnostic investigation is necessary, since numerous systemic diseases are associated with the syndrome, such as acute pancreatitis, leukemias, gas, fat or amniotic emboli, lupus , among others, being necessary the treatment of the specific disease.
