CASE REPORT OF ISCHEMIC RETINOPATHY WITH PRESUMED FEVR
The purpose of this study is to report a case of unilateral peripheral retinal ischemia in a young patient with no history of prematurity and other previous diseases.
Presentation of a clinical case
Woman, 29 years old, with complaint of worsening visual acuity for 20 days. In the
medical appointment she presented partial visual acuity 20/30 in the right eye and
20/20 in the left eye. She denied personal and family ophthalmological antecedents
and previous diseases. At funduscopy, peripheral vascular closure was present in the right eye,
with presence of phantom vessels, temporal and nasal peripheral retinoschisis, distortion of
vascular arches and posterior pole hemorrhage.
Laboratory tests were performed to exclude sickle cell anemia (hemoglobin
electrophoresis and sickle cell pattern) and diabetes (glycemia 85), and the patient’s
gestational age of 41-weeks- old at birth was confirmed.
For the treatment, two laser photocoagulation sessions were performed on the
peripheral avascular retina and the final visual acuity of the right eye got to partial
20/25.
It is concluded through this work that although the disease presents an important
genetic component, there is a part of the population that does not have a positive
family history and it is necessary to exclude other diseases such as diabetic
retinopathy, retinopathy of prematurity and retinopathy of sickle cell anemia, resulting
in diagnosis by exclusion. With this report, it was observed the importance of early
diagnosis associated with the correct therapeutic approach, which avoided the serious
complications of the disease and maintained a good visual acuity to the patient.
