CASE REPORT: BEHÇET'S DISEASE WITH RETINAL MANIFESTATION

Introdução / Purpose

Behçet's desease is a rare sistemic inflamatory vasculitis. It can afect any system, and ocular manifestation are comon.The prevalence 0,6 to 370 cases per 100000 acording to the population.
Ocular, and retinal are on the International Classification Criteria Of Behcet's Disease.

Material e Método / Methods

Case report of a 28-year-old female patient attending the emergency department of the Instituto Penido Burnier

Resultados / Results

NNR, famale 28-year-old, white, married
Comes to the emergency department with complaint of sudden loss of visual acuity in the right eye.
She claims being under investigation for behçet's disease and is using prednisone 50mg/day for 2 days
At the ophthalmologic examination the patient had best visual acuity on the right eye minus than 20/400 (counting fingers at 30cm) and in the left eye 20/80. The IOP was 12 at OD and 15 at OS. The Biomicroscopy the right had Flare +/4+, anterior chamber reaction 2+/4+ and mobile hipopion the left eye had no flare and +/4+ anterior chamb reaction. The Fundus exacmination on the right eye showed macular edema,no sheathing of vessels, cottony exudates in the territory of the upper temporal artery and hyperaemia of the optic nerve, the left eye showed decreased macular reflex, temporal pigmented scar, cottony exudates in the territory of the upper temporal artery.
The patient do not tolerate more than 50mg/day of oral prednisone due to nausea. Topical steroids were prescribed and the patient was referred for clinical evaluation. Was admitted to another service where pulse therapy was performed and returned after 5 days with partial improvement of vision and examinations were performed: macular OCT, fluorescein angiography and retinography.

Discussão e Conclusões / Conclusion

Due to the rarity of Behçet's disease, the ophthalmologist's close attention to the diagnosis and adequate clinical follow-up of the patient is important
