OCTA FOR DIAGNOSIS OF CHOROIDAL NEOVASCULARIZATION BY BILATERAL CENTRAL SEROUS CHORIORETINOPATHY AND EXUDATIVE AGE MACULAR DEGENERATION: CASE REPORT

Introdução / Purpose

Report a choroidal neovascularization secundary to bilateral central serous chorioretinopathy as differential diagnose to exsudative age macular degeneration.

Material e Método / Methods

OPTICAL COHERENCE TOMOGRAPHY, OPTICAL COHERENCE TOMOGRAPHY ANGIOGRAPHY AND FLUORESCENTE RETINOGRAPHY

Resultados / Results

Male patient, caucasian, 70 years old, was fowarded to a retina evaluation pre phacoemulsification surgery and in the ophthalmologic examination, he presented corrected visual acuity of 20/30 in the right eye and 20/40 in the left eye . In the biomicroscopic examination there was a nuclear and cortical opacification and intraocular pressure of 13 mmHg in both eyes. Indirect binocular ophthalmoscopy visualised a macular edema and some peripheral drusen in both eyes. The fluorescente retinography showed a hiperfluorescence due to a window defect and central leakage in both eyes (photo 1 and 2). The optical coherence tomography showed pigment ephitelium detachment with serous detachment and the OCT angiography showed a large fedder vessel in both eyes (photo 3 and 4)

Discussão e Conclusões / Conclusion

Central serous chorioretinopathy (CSC) is an idiopathic and multifactorial disease characterized by a serous detachment of the sensorineural retina at the posterior pole due to the accumulation of serous fluid between the sensorineural retina and retinal pigment epithelium (RPE). It often affects more men in the 4-5th decade of life. It is very associated with the use of corticosteroids, endogenous hypercortisolism and emotional stress. Usually, the acute form is self-limited with spontaneous resolution in about 3-4 months. However, the chronic form may present some complications, such as choroidal neovascularization, cystoid macular edema, sub-retinal lipid deposits and choriocapillar atrophy, which should be identified and treated early, as they may cause severe and irreversible central visual loss
