AZOOR - A REPORT CASE WITH MULTIMODAL APPROACH

Introdução / Purpose

Acute zonal occult external retinopathy (Azoor)) was first described in 1993 as a syndrome with rapid loss of areas of the external segments of the retina.
It is characterized mainly by acute photopsies and scotomas in the visual field, in addition to fundoscopic alterations and electroretinographic abnormalities, affecting one or both eyes.
The etiology of AZOOR remains controversial. Gass suggested an external viral retinal infectious process. Autoimmune and inflammatory hypotheses were also proposed. Other possible mechanisms include fungal infiltration, polycythemia vera, toxic retinopathy, antiretinal antibody.
We report a case of Azoor with a multimodal approach and macula spare.

Material e Método / Methods

A.L., male, 35 years old, with Crohn's disease, with eye pain in the right eye associated with nyctalopia, without other signs or symptoms. In regular use of mesalazine and azathioprine. The patient denies other comorbidities, surgeries or eye trauma. At ophthalmologic examination, the patient had a better visual acuity 20/20 J1. Biomicroscopic examination without relevant changes. Intraocular Normotension, fundoscopy with hyperpigmented lesions in the inferior temporal arch of the right eye, accompanying local vasculature.

Resultados / Results

Complementary exams showed: - OCT-A: preserving of the retinal layers, the foveal anatomy, and the vitreoretinal interface in the macular region of both eyes, preservation of the macular vascular plexuses, and absence of lesions in all the studied layers.; Autofluorescence: retinal structures are preserved in the macular region, but with hypoauthofluorescence in the region of the lesion by pigmentary block.

Discussão e Conclusões / Conclusion

Azoor is a rare disease with poorly understood physiopathology. We describe a case that occurred in the peripheral region and with an associated autoimmune disease. We believe that further investigation is necessary to verify the etiology of this disease and the association with specific diseases.
