OCULAR CHANGES CAUSED BY SILICONE OIL IN PATIENTS WITH PREVIOUS RETINAL DETACHMENT AFTER 6 MONTHS FOLLOW–UP

Introdução / Purpose

The purpose of this study was to evaluate the anatomical and functional results, as well as the complications related to the use of silicone oil in a specialized service in vitreoretinal surgery, linked to the Unified Health System of São Paulo, Brazil (SUS).

Material e Método / Methods

Retrospective study using medical records of patients submitted to removal of silicone oil (SO – 5000 cs) from the vitreous cavity and anterior chamber (AC), when necessary, in a tertiary care service of SUS. We included all cases of SO removal, with 6 months postoperative follow-up.
Surgical indications of SO injection were separated into 2 groups: (1) Rhegmatogenous retinal detachment (RRD); (2) Tractional retinal detachment (TRD).

Resultados / Results

One hundred and twenty-two medical records were analyzed and 94 eyes were included.
Group 1 included 45 eyes and group 2 total of 49 eyes. Intraocular pressure (IOP) greater than 25 mmHg was found in 18 eyes (40%) (group 1) and 14 eyes (28.5%) (group 2).
SO emulsification was detected in 17 eyes (37.8%) (group 1) and 11 eyes (22.44%) in group 2, 20 eyes (44.5%) had SO in the AC X 12 eyes (24.5%) in group 2, SO detection in the AC was more evident in cases with emulsified SO and it had a significant difference for both groups (Fisher's exact test; p = 0.0001).
Pale optic disc was found after SO removal in 19 eyes (42.2%) (group 1) and 31 eyes (63.26%) (group 2).

Discussão e Conclusões / Conclusion

Compared with the literature, our study shows excellent anatomical success but with a number of complications similar or even worse in the cases of SO indicated for complex rhegmatogenous retinal detachment (proliferative vitreoretinopathy ≥ C1) and a longer permanency of SO in vitreous chamber. Statistically significant result was found when there was emulsified SO in vitreous cavity and its presence in the anterior chamber.
