CHOROIDAL VASCULARIZATION (CNV) DUE TO PUNCTATE INNER CHOROIDOPATHY TREATED WITH BEVACIZUMAB: A CASE REPORT
Introdução / Purpose
To report a case of a female patient which presented contuse trauma in the right eye (OD) and developed loss of vision in both eyes (OU) in despite of multiple anti-VEGF injections.
Material e Método / Methods
Case report.
Resultados / Results
A 40-years-old female patient complained of vision loss in the left eye (OS) for 8 months. The OD had loss of vision for 8 years after a blunt trauma. Ophthalmological history was positive for prior 6 anti-VEGF injections on OS for sub-retinal active neovascular membrane. Medical history was positive for hepatitis C. Best corrected visual acuity was counting fingers on OU with a -3.00 SD optical correction. Biomicroscopy of the anterior segment was unremarkable. Fundus examination showed a large scar concentric to the optic nerve with macular involvement on OD and a fibrotic macular scar on OS, besides multiple round small hyper and hypopigmented lesions in the posterior pole of OU (Image 1). Fundus fluorescein angiography (FFA) showed staining (Image 2). OCT revealed sub-macular fibrosis with no sub-retinal fluid (Image3). Punctate Inner Choroidopathy in both eyes with treated choroidal neovascularization was diagnosed.
Discussão e Conclusões / Conclusion
Punctate Inner Choroidopathy is a disease which affects mostly young women with myopia, bilaterally. It occurs with multiple small yellowish lesions in the posterior pole without vitreitis. Choroidal neovascularization may happen as a complication in 40% of the cases which has reserved prognosis, as in this case. Relapsing of the CNV is common.
