SUBRETINAL NEOVASCULAR MEMBRANE DUE TO PRESUMED OCULAR SARCOIDOSIS

In this case report, we describe the case of a patient diagnosed with Presumed Sarcoidosis from the ocular investigation of a Chorioretinitis with neovascular subretinal membrane

Female, 29 years old, presented with sudden low visual acuity (VA) in the left eye (OS) one month before visiting the general ER and was diagnosed with neuroretinitis by Bartonella sp. At inicial ophthalmological examination VA was 20/25 in the right eye (OD) and 20/250 in OS. A fundus examination revealed a subretinal lesion and retinal wrinkling in the macular area in OS. Optical coherence tomography (OCT) revealed neovascular subretinal membrane in OS. Blood tests were negative to Bartonella sp., syphilis, HIV, toxoplasmosis, CMV, toxocariasis, HSV and tuberculosis. During the follow-up, a fundoscopy revealed Dallen Fuchs nodules in both eyes (OU), that combined with chest X-ray findings and high serum level of angiotensin‐converting enzyme (ACE) lead to the hypothesis of sarcoidosis and she started treatment with immunosuppressants.

Discussão e Conclusões / Conclusion

Sarcoidosis is a multisystem chronic inflammatory disease of unknown etiology, that can involve any organ or tissue. However variable among published clinical reports, the frequency of ocular involvement in patients with sarcoidosis can be estimated between 30 to 60%. Those ocular manifestations may develop in the absence of any apparent systemic involvement and may even be the main or sole site of the disease, leaving to the Ophthalmologist the responsibility to suspect and diagnose this disease, what can be a challenging task, once all ocular structures may be involved in a variable pattern.
