MULTIMODAL IMAGING IN CASES OF PRESUMED OCULAR TUBERCULOSIS: A SERIES OF CASES

Introdução / Purpose

This series of cases reports three cases of presumed ocular tuberculosis associated to fundoscopic alterations, through retinal examination, widefield retinography, autofluorescence and angiofluoresceinography; optical coherence tomography (OCT) and OCT angiography (OCTA). The first case deals with a 63-year-old woman with visual acuity loss in a single eye, recent history of pulse therapy for thyroid disease, associated to multifocal choroiditis, IGRA was positive and there was cavitary lesion at the apex of the lung. The second case involves a 47-year-old female with low visual acuity in both eyes for 6 months, she had a prior suspicion of polypoidal vasculopathy, undergoing intra-vitreous anti-angiogenic therapy. At examination, he presented placoid lesion at the posterior pole and subretinal neovascular membrane, positive IGRA and pulmonary cavitation lesion. The last case was a 29-year-old male patient, with a report of low visual acuity in the right eye for 20 days, coursing with retinal vascular branch oclusion. At fluorescein angiography, he also had vasculitis, ischemic areas and retinal neovascularization. The IGRA test was positive.

Resultados / Results

It was possible to analyze different fundus images using different imaging methods combined with clinical data, laboratory and radiological exams to reach the etiological diagnosis. All three patients progressed favorably after the institution of appropriate treatment.

Discussão e Conclusões / Conclusion

Tuberculosis can manifest itself in a variety of ways. The ocular form is one of the manifestations of systemic tuberculosis. This case report highlights the importance of adequate diagnosis and institution of specific treatment to avoid permanent visual loss.
