INTRACRANIAL HYPERTENSION CASE DIAGNOSED BY PAPILLARY EDEMA IN ASSOCIATION WITH THE ULTRASOUND EXAMINATION
Introdução / Purpose
The main purpose of this study is to demonstrate a intracranial hypertension case diagnosed by papillary edema in association with the ultrasound examination.
Material e Método / Methods
The fundus images were performed with the wide-field retinal imaging system to record papilledema and ultrasonography to evaluate the most common diagnostic hypotheses.
Resultados / Results
Patient complaining of headache and visual alteration, without improvement with the migraine treatment prescribed by a neurologist, presented ophthalmological examination diplopia, mydriasis and papilledema.
Bilateral papilledema was demonstrated on retinography, with no further alterations.
Ultrasonography confirmed papilledema. Due to the finding, the 30-degree abduction test was performed, in which the decrease of the fluid can be evaluated, leading to the diagnosis of intracranial hypertension. After lumbar puncture, the patient improved, reducing complaints.
Discussão e Conclusões / Conclusion
This case demonstrates the importance of anamnesis associated with ophthalmologic examination, and in cases of suspected papilledema, the use of ocular ultrasound is essential for the differential diagnosis and evaluation of retrobulbar optic nerve.
