A CASE REPORTED OF LYME DISEASE DIAGNOSED IN EYE BANK FOUNDATION OF GOIAS

Introdução / Purpose

Lyme disease is an infection transmitted by arthropods (Ticks Ixodes, sp.), Caused by the spirochete Borrelia burgdorferi. It presents clinical manifestations that can be classified in 3 stages with neurological, cardiac and ocular involvement among others.
This report is a patient with a history of travel to an endemic region of the disease that presented primarily ocular manifestations being diagnosed in Brazil at Fundação Banco de Olhos de Goias(FUBOG).

Material e Método / Methods

It consists of a case report of a female patient, 28 years old, from the state of Goiás, single, call girl. He sought FUBOG for about 4 months with low visual acuity in both eyes, with a previous pathological history of bilateral uveitis about 1 year and 6 months without systemic manifestations. It referred to constant foreign travel, both for Europe and the USA.
The ophthalmologic examination revealed a visual acuity of 0.2 in both eyes and a moderate anterior chamber reaction was observed in addition to clear signs of neuritis, vasculitis with increased vitreous cellularity.

Resultados / Results

The main IGG and IGM serologies were negative, deciding to investigate other diseases, highlighting the epidemiology of the regions frequented. Among the results obtained were the Western blot IGM positive with high titres for Lyme disease. Early laser and antibiotic treatment (ceftriaxone 2g IV 21 days) was instituted with improvement of visual acuity to 1.0 OD and 0.4 EO.

Discussão e Conclusões / Conclusion

Ocular involvement may be present at any stage of the disease and all ocular structures may be involved. In this case we could observe a severe uveitis that was promptly controlled with the clinical treatment, with laser and antibiotic (ceftriaxone 2g IV 21 days) evolving with improvement of visual acuity to 1.0 OD and 0.4 OE.
In general Lyme disease is self-limiting, having a good prognosis with complete recovery in the earliest stages
