AGGRESSIVE BILATERAL VISUAL LOSS IN A ADOLESCENT

Summary

A 16-year-old male patient presented in our clinic with low visual acuity in the right eye for 7 days. He had a similar complaint in the left eye eight months ago, with persistent visual loss. Past ocular history and past medical history were unremarkable. The ophthalmological exam showed BCVA of CF 3m in OD and CF 30cm in OS. The slit-lamp exam had normal findings except for 1+ vitreous cells in both eyes. Fundus examination had whitish subretinal lesions diffusely in the posterior pole and confluent subretinal fibrosis that encompassed the optic nerve head trough the macula. The fluorescein angiogram had hyperfluorescent dots that both stained and leaked in OD and a subretinal lesion in the posterior pole that stained surrounded by hyperfluorescent dots. Serologies for HIV, syphilis, hepatitis B and C were negative and the tuberculin test was 0mm. The diagnosis was presumed as Progressive Subretinal Fibrosis and Pan-uveitis. This is the second report in Brazil and the youngest patient with the syndrome in the country.
