OPTICAL COHERENCE TOMOGRAPHY ANGIOGRAPHY (ANGIO-OCT) IN THE INITIAL DIAGNOSIS OF IDIOPATHIC POLYPOIDAL CHOROIDAL VASCULOPATHY: CASE REPORT

Introduction

Idiopathic polypoidal choroidal vasculopathy is a vascular disease of the choroid in which aneurysmatic dilation and formation of vascular polyps occurs. It is more common in patients of oriental origin than in whites. It is predominant in women with an average age of 60 years. It is usually presented bilaterally, but asymmetrically. The aim of this study is to report a case of use of Angio-OCT for initial diagnosis of Polypoidal Choroidal Vasculopathy (PCV).

Methods

Use of Angio-OCT Topcon for initial evaluation and diagnosis of Polypoidal Choroidal Vasculopathy, followed by diagnostic confirmation with Green Indocyanin Angiography.

Results

79-year-old male, white, with a history of acute visual impairment with central scotoma and metamorphopsia. On ophthalmological examination on August 26, 2019, he presented visual acuity (AV) with the best correction of 0.40 in the right eye (OD) and 1.00 in the left eye (OE). Fundoscopy altered macular luster, with an orange-colored and round shape lesion in the right eye. Physiological in left eye. He performed OCT by Swept Soure (SS) on the same day, the presence of serous detachment of the sensorial retina associated with the breakdown of the Retinal Pigment Epithelium with subretinal hyperreflectivity temporally to the fovea, in addition to pachycoroid. On September 2, 2019, Angio-OCT en face was performed, revealing multiple vascular polyps temporally to the foveal topography and in a papillomacular bundle in a coriocapillary section. Subsequently, on September 4, 2019, Indocyanin Green Angiography was performed, evidencing a juxtafoveal hot spot and in the papillomacular bundle confirming the diagnosis of Idiopathic Polypoidal Choroidal Vasculopathy.

Discussion

Optical Coherence Tomography Angiography (Angio-OCT) has been shown to be able to detect vascular pathologies in the macular region, mainly related to choroidal neovascularization and now also in the detection of choroidal polyps in Idiopathic Polypoidal Choroidal Vasculopathy.
