CASE DESCRIPTION: Child, female, 6-month-old, with presence of nystagmus and difficulty following objects. Twin pregnancy without infection, use of teratogenic medications or trauma. Twin pregnancy delivered by cesarean section. Born at 37 weeks of gestational age , with 2,130 kg birth weight . Mother denies any history of eye disease in family . Under light stimulus, difficulty following light. Examination under sedation performed in the operating room. Slit-lamp biomicroscopy showed no anterior segment changes in both eyes, only temporal opacity in the anterior vitreous, and tonometry was 14 mmHg. Fundus examination showed macular dragging with temporal retinal detachment, retinal pigment epithelium thinning, and fluorescein angiography, normal perfusion of retinal vessels at the posterior pole, hypofluorescence by peripheral hypoperfusion, and peripheral focal leakage hyperfluorescence in both eyes. Similar findings were found in sister. After consent signed by the mother, and considering the areas suggestive of hypoperfusion and neovascularization on angiography, laser photocoagulation was indicated in the ischemic areas. We guide the investigation of eye diseases in family members.
