ABNORMAL RETINAL MASS

Summary

Case presentation: A 63-year-old woman with myopia, past ocular history of LASIK, PPVV + Scleral Buckle for RD + MH, with past history of breast cancer in 2012 and systemic hypertesion, who suffered an acute visual loss in her left eye after a severe vomiting episode ten days ago. On examination her visual acuity was 20/25 in her right eye and 20/100 in left eye, pupils, intraocular pressure, and anterior segment examination were normal, while dilated examination demonstrated a pigmented elevated bilobulated lesion at the superotemporal equator, within the scleral bucke limits in her left eye, extending into the macular region. Left eye ultrasound examination showed elevated mass with irregular A scan and could not rule out the hypothesis of melanoma. A more conservative approach was adopted and we observed the patient, who returned 15 days later with a minor lesion, measured with US. After 90 days, complete regression of the injury was observed. Discussion: This is a rare case of Spontaneous suprachoroidal hemorrhage secondary to valsalva maneuver. valsalva maneuvers and systemic arterial hypertension are predisposing factors for choroidal vessel ruptur. In the literature there are cases associated with the presence of scleral buclke. However, spontaneous resolution is common.
