UNILATERAL DECREASE IN ACCOMMODATION IN A YOUNG PATIENT AFTER BARRIER LASER ON PERIPHERAL RETINAL TEARS

Summary

BAM, female, 16 years old, comes for a routine consultation referring to progressive worsening of visual acuity. Patient wearing glasses and soft contact lens, without previous eye surgery or procedures, with myopic father and mother. On examination, using in the right eye: -1.25 (ESF) -0.25 (cil) 170 and in the left eye: -1.50 (ESF) she presented visual acuity of 20/20 away in the Snellen table and at 33cm he read J1 in the Jagger's table. During retinal mapping, retinal ruptures were observed throughout the temporal periphery in both eyes. Barrier laser performed on both eyes under mydriasis, without complications, blocking all ruptures. 2 days later, she returned to the service reporting difficulties with close vision. The symptom started after the laser procedure. On the exam, her visual acuity for distance was 20/20 wearing her glasses. The patient was able to read J1 at 33 cm with her left eye and needed an addition of + 2.00D on the corrective lens to read J1 with her right eye. Biomicroscopy showed a photoreagent pupil in the right eye, but with a decreased pupillary reflex in relation to the left eye. Treatment with hormonal anti-inflammatory was performed every 8 hours for 7 days, with no improvement in the condition. Accommodative disorder still under investigation.
